Why It Is Classified as Rare

  • Low Overall Prevalence: There is no massive widespread data because idiopathic (spontaneous) dislocation in healthy individuals is exceptionally unusual.

  • High Fatality Rate: Many traumatic cases are unfortunately never counted in living prevalence statistics. Because a dislocation at the C1-C2 level compresses the brainstem and upper spinal cord, severe high-impact trauma often leads to immediate respiratory arrest or sudden death.

  • Selective Sub-Populations: AAD is primarily seen as a rare secondary complication linked to specific underlying conditions rather than a standalone illness.

Common Root Causes of AAD

When AAD does occur, it is typically triggered by one of three pathways:

  • High-Energy Trauma: Severe car accidents, falls from significant heights, or extreme sports injuries that physically tear the stabilizing transverse ligaments in the neck.

  • Congenital Skeletal Conditions: Some individuals are born with malformed neck bones or hyper-flexible ligaments. It is heavily associated with Down Syndrome (where up to 30% have radiographic joint laxity, though only 1-2% develop symptomatic dislocation) and rare bone dysplasias.

  • Inflammatory / Autoimmune Diseases: Severe, long-term Rheumatoid Arthritis (RA) can erode the bones and ligaments at the base of the skull over time, causing the joint to slip out of place.
